1. I confirm all the details are correct.
2. Policybazaar (including its representatives) shall not be held liable for any changes due to incorrect information.
3. I/We hereby authorise the Company to Collect , use and store my/our sensitive personal data or information and to disclose the same to any third party as the Company may require, in connection with the life insurance business, and/or to comply with the applicable laws, rules and regulations, including anti-terrorism, anti-money laundering and tax reporting rules and regulations.
4. Applicable for market linked plans - I understand that the returns in ULIP plans are subject to market risk and are not guaranteed. The investment risk in the policy is borne by the policyholder. The actual returns can vary depending on the performance of the chosen fund, charges towards mortality, allocation, policy admin, cost of riders, etc. I have read and understood the benefit illustration with 4% and 8% returns and understand that it is neither the minimum or maximum limit that I may get as a policyholder.
   I agree and authorize the Insurance Company to share the portfolio/fund details pertaining to my policy/policies held with the Company with Policy Bazaar and authorize Policy Bazaar to store the details.
